BLH's Population Health Initiative works at the system level, partnering with managed care organizations (MCOs), hospitals, and physician practices to identify at-risk members and deliver coordinated, evidence-based interventions before conditions escalate.
By addressing social determinants of health alongside clinical needs, our approach reduces unnecessary emergency room visits, observation stays, and inpatient admissions, improving outcomes across entire populations.
