Beatrice Loving Heart’s Adult Community Services team supports individuals with disabilities and complex needs to help them achieve optimal quality health, affordable housing, and meaningful community life. We work with adults who live in the community and want to remain independent and others who live in a Maryland nursing facility or are homeless and wish to move to a home. Our highly trained Housing Coordinators, service coordinators, and Health Coaches navigate complex systems and connect individuals with the essential supports and services to thrive at home. With our specialized person-centered planning abilities, we work with each individual to customize a care plan centered on attaining personal goals and aspirations. Our team works together to:

  • Identify and secure affordable and accessible housing, provide housing supports, and resources
  • Assist with the transition from a nursing facility to a home in the community
  • Develop a person-centered plan based on needs, preferences, and goals to help ensure health and safety and facilitate community inclusion
  • Assist with obtaining environmental assessments, home-delivered meals, and assistive technology
  • Help coordinate medical appointments and assist with obtaining durable medical equipment and medical supplies
