Children & Youth
Beatrice Loving Heart partners with families and children and youth with special health care needs to navigate complex systems and connect individuals with the necessary supports and services to thrive at home and in their communities. Our highly-skilled, licensed professionals include Doctors, Social Workers, and registered nurses who understand that each family and child are exceptional. Using a person-centered planning approach, we work with each individual and family to tailor a plan of care centered on attaining personal goals and aspirations. The ways that we support are as follows:
- Assist with coordinating and participate in medical appointments
- Provide community resources
Attend and coordinate IEP meetings - Ensure communication with and among the medical home team is clear, frequent, and timely
- Support, prepare and plan for all transitions, including early intervention to school, hospital to home, pediatric care to adult health care
- Educate children, youth, and families on self-management skills
